The only fault I've experienced in all the time I've been playing both versions was a broken coupler - through no fault of my own but just a random failure.
There was a website in the early 2000s (still there, I just checked) called "Tales From the Krug", a photographic diary compiled by Al Krug, an engineer with BNSF based at Sheridan.
I just remembered a tale he told about a broken coupler ......
